FMA will occasionally offer the field carpet after regular season and off-season events. Use this form to submit your interest in securing a portion of the field carpet. 

Overall Notes about the process:

1. The carpet is available only at the end of the event after field tear down (1-3 hours after conclusion of the event). The carpet will not be stored at the venue or transported in PODS for future pickup. 
2. Adequate personnel from the recipient team must be present to roll and remove the carpet. Field personnel and volunteers from the host teams will not be available to assist with this process nor will any equipment for rolling, moving, or transporting the carpet will be made available. It is also preferred that the recipient team and personnel assist with field tear down. Please note that the field carpet will not be available until after permission is granted from field personnel. This is to ensure that the field material is adequately removed and collected to avoid loss and ensure successful future events.
3. Confirmation prior to the event and checking in with Field Personnel at the event is requested to verify the recipient team is present and available to remove the carpet.
4. The carpet is 15’ wide and a full roll is about 400 lbs. In most cases, the carpet is cut in half and offered to two to four teams.
5. FMA teams will take precedence over non-FMA teams.

Process for Requesting Field Carpet:

1. Fill out the Field Carpet Interest Form. Note: this is not a guarantee of field carpet availability. It is also recommended to fill the form out once per season if carpet was not provided. 
2. Field carpet is offered on a first come, first served basis based on the order this form is filled out and prompt responses.
3. FMA personnel will contact you if carpet is available based on your response from the Interest Form. If there is a slow response, the field carpet may be offered to another team.
